Notes: this I-94 contains some common errors, including the following:

1. The employee has not entered his complete address or A#

2. The employer has improperly obtained more documentation than the law permits. The employee should provide either a document for List A or a document from List B and List C. Requiring more documentation than the law permits is not being careful, it being discriminatory! Also, even if the Employer omitted the information under List A, the documentation under Lists B and C are insufficient as it omits the required information concerning the issuing authority, document # and document expiration.

3. The employer has omitted its name and address.
